Service Summary
Hydrotherapy in a heated therapeutic pool, using buoyancy, water pressure and warmth to make movement easier and less painful than exercising on land.
Possible Side Effects
Hydrotherapy works through the physical properties of water rather than through any drug or device, and it is worth understanding the mechanism because the same properties that make it useful are what create its risks. Buoyancy carries part of your body weight, so joints are loaded less and a weak movement can be assisted, counterbalanced or resisted simply by changing how you move against the water. Hydrostatic pressure acts uniformly on the immersed body, which supports the circulation and helps disperse swelling. Warmth eases muscle guarding and reduces pain by acting on the thermal and mechanical receptors, lowering the nociceptive input from the treated area. Immersion is therefore a real physiological event, not a gentler version of a land session, and the safety picture belongs to water specifically. Current UK professional guidance (ATACP, 2021) is explicit that many conditions once treated as barriers can be managed safely in a well-run pool by a physiotherapist who understands both the pathology and the effects of immersion, with a risk-benefit assessment made for the individual patient. It also names conditions where aquatic physiotherapy is not to be considered at all: diarrhoea or vomiting within the previous 48 hours; medical instability after an acute event such as a heart attack, stroke, deep-vein thrombosis or pulmonary embolism, without medical clearance; uncontrolled or severe cardiac failure; unstable angina; uncontrolled epilepsy, uncontrolled diabetes or unstable blood pressure where there is no medical management plan; an acute systemic illness with fever, where a clear day is required before entry; and body weight above the limit set by the pool’s emergency-evacuation assessment. A second group of conditions are precautions rather than absolute bars, and they need to be declared in advance so the session can be planned around them rather than cancelled at the poolside: open, surgical or healing wounds, which are covered with a waterproof dressing and which mean postponement if a wound is actively bleeding or heavily exuding; faecal or urinary incontinence, managed with a bowel programme, incontinence swimwear, bladder voidance before entry, or a catheter left in situ and strapped; a tracheostomy, which is managed under dedicated specialist guidance; controlled epilepsy, which needs an individual seizure plan and enhanced screening; and chlorine sensitivity, which current guidance treats as a precaution and older guidance treated as an absolute bar. If you are pregnant, or wear a transdermal medication patch, ask for specific advice before booking, because both carry an upper water-temperature limit. The side effects patients actually experience are usually mild and mechanical. Leaving a heated pool removes hydrostatic pressure abruptly and blood pressure falls further on exit, so dizziness or light-headedness as you get out is common, and supervision on leaving the water is standard practice. The heat itself is tiring: drowsiness afterwards is normal, and while you feel it you should not drive or operate machinery. Immersion also increases urine production at the same time as sweating stops working, so dehydration is a genuine risk — drink afterwards and allow a rest period for your temperature to settle. Chloramines in the water can cause skin or eye irritation, usually appearing within 24 hours. Eating a couple of hours before your session reduces the chance of dizziness or fainting, you should report any new infection, fever or open wound before you enter, and you should tell the therapist immediately if you feel unwell in the water. In the trials that have examined this most closely, including a Cochrane review of aquatic exercise in knee and hip osteoarthritis, no serious adverse events were reported in relation to aquatic exercise.
